  3. If you are constructing a new home (or development), you will need to opens in a new windowDownload our Construction Application hereopens PDF file .
  4. You will need a copy of:
    a.  Your lease agreement or escrow closing document.
    b.  Driver’s License
    c.  Applicants for service will be subject to an online soft credit screening. A deposit according to MID’s Fee Scheduleopens PDF file may be required to activate service.
  5. Return the completed Residential Service Application along with the required documents to MID.
